Success Rate
Upgrades to items are based on the chance of success, and as with any random system there is an opportunity that the upgrade won't succeed. There are several ways to increase your chances of success. This includes the use of an Upgrade Pity or the addition additional materials to your upgrade.
The Upgrade Pity is available from the NPC for the smithing process in Lost Ark. It increases the odds of an upgrade's success by some percentage. It's similar to the "luck bonus" that some items provide, but it is more solid. It can be added to any armor or weapon, and can be used up to three times.
You can also add additional materials to the upgrade to increase the chances of success. These materials are displayed in a window above the + button. There are some limitations. The maximum amount of material you can add is shown directly above the + button.
A Blacksmith Blessing or an upgraded version thereof, can be used to boost the likelihood of a successful upgrade.
